What Qualifies a Child...

What qualifies a child for assistance from A Child's Path Back? To be a qualifying child you must meet the following criteria:

Qualifications of Children:

Manner in which a Family is Advised of A Child's Path Back's Services:

Schools will pass information to parents through flyers
Schools will pass information to parents once notified of a qualifying event of a child
Flyers and information brochures will be available in the school office
Hospital staff will hand out flyers and notify the patient family
The Shrine will send out information brochures or flyers to the family of an injured child
Real Estate agents will update potential residents of the program available
Synergy Services will hand out flyers to those found to qualify

Advocate / Back to School Process Summary:

Family is made aware of A Child's Path Back Services as stated above and contacts A Child's Path Back, Inc. for assistance
A Child's Path Back, Inc. contacts the local fire department and assigns the child an advocate
The Advocate gets updated local resource guide to the family and has the release of liability signed and forwards that to A Child's Path Back, Inc.
A Child's Path Back contacts hospital staff, local resource service, and Synergy Services to set up a meeting with all agencies applicable including the advocate and the family
An action plan is developed by members of the above along with school personnel
Family approves plan, declines plan, or requests adjustments to the plan
Approved plan is implemented at the school or community group prior to the child returning
Follow up and reporting to A Child's Path Back is completed by the advocate
If additional follow up or resources are needed by the child and family, the advocate refers the information back to A Child's Path Back
A Child's Path Back obtains the needed resources, information, etc. to refer the family on for additional services needed

Any medical complication that is life altering or life changing in nature
Any Traumatic Brain Injury resulting in decreased mobility, physical ability, and/or
Mental status change that alters the child's day to day functions
Any traumatic injury resulting in the loss of a limb
Any injury resulting in visible scarring that is found to cause embarrassment, change
In life style, and/or normal day to day functions
Any affect of an illness that meets any of the above criteria
Any child being relocated to a new school district or community group where the
Child is not known and meets any of the above criteria from a previous injury or
Illness
